Please note the following:

  1. Supporting documents (Step 2 of the application process) are no longer available for download.  They will be available in the near future due to revisions.
  2. If you downloaded a copy of the supporting documents packet before January 22, 2009, you will have to download the REVISED packet at a later date.
  3. All applicants must submit the REVISED Supporting Documents packet.
  4. Please check nhhsp.org regularly.  The REVISED Supporting Documents packet will be made available ASAP.
